| Abstract: | A logística no transporte de órgãos para transplante é uma das operações mais críticas
e complexas da gestão de suprimentos contemporânea, onde a variável tempo é o
determinante direto entre a vida e a morte. O presente trabalho teve como objetivo
analisar os desafios logísticos enfrentados pelo Sistema Nacional de Transplantes
(SNT) no Brasil, com foco na eficiência dos modais de transporte e no impacto do
Tempo de Isquemia Fria (TIF) na viabilidade dos enxertos. A medida adotada consistiu
em uma pesquisa bibliográfica e documental, com abordagem qualitativa, utilizando
se de dados oficiais e literatura especializada. Os resultados indicam que, embora o
Brasil possua um dos maiores sistemas públicos de transplante do mundo, a extensão
territorial e gargalos na infraestrutura de transporte ainda representam obstáculos
significativos. Conclui-se que a integração entre a Central Estadual de Transplantes
(CET) e as companhias aéreas, aliada ao uso de tecnologias de conservação, é
fundamental para a redução do descarte de órgãos e para o sucesso dos
procedimentos cirúrgicos.

Logistics in the transport of organs for transplantation is one of the most critical and complex operations in contemporary supply chain management, where the time variable is the direct determinant between life and death. The present study aimed to analyze the logistical challenges faced by the National Transplant System (SNT) in Brazil, focusing on the efficiency of transport modes and the impact of Cold Ischemia Time (CIT) on graft viability. The adopted methodology consisted of bibliographic and documentary research, with a qualitative approach, using official data and specialized literature. The results indicate that, although Brazil possesses one of the largest public transplant systems in the world, the territorial extension and infrastructure bottlenecks still represent significant obstacles. It is concluded that the integration between the State Transplant Center (CET) and airlines, combined with the use of preservation technologies, is fundamental to reducing organ disposal and ensuring the success of surgical procedures.
