Assistência de enfermagem em cuidados paliativos

Abstract

O câncer é uma patologia caracterizada por um crescimento celular desapoderado que afeta órgãos e tecidos do corpo. O agravamento do caso clínico oncológico é denominado de metástase, onde as células cancerosas espalham-se para outras regiões do corpo. Como o câncer é uma doença com alta taxa de letalidade, há uma grande possibilidade de que o tratamento paliativo se torne uma melhor opção para benefício pleno do cliente oncológico e de seus familiares, assim que a impossibilidade de cura é apresentada. Este trabalho procura identificar as melhores técnicas assistenciais que podem ser adotadas pela equipe de enfermagem para proporcionar um tratamento humanizado e holístico nos cuidados paliativos, destacando-se a promoção do bem-estar e qualidade de vida do paciente e familiares. A partir da coleta e revisão de artigos bibliográficos levantados nas plataformas LILACS (Literatura Latino-Americana e do Caribe em Ciências da Saúde), BVS (Biblioteca Virtual da Saúde) e SciELO (Scientific Electronic Library Online), que abordam a enfermagem nos cuidados paliativos na oncologia adulta publicados no período de 2003 a 2022 na língua portuguesa. Foram identificados 13 estudos que se alinham na construção de uma assistência voltada ao conforto e dignidade do paciente, assim como, a importância de incluir os familiares neste tratamento, abordando algumas técnicas de cuidados paliativos que a equipe de enfermagem pode exercer no ambiente hospitalar. Em contraste, também foi identificado a falta de preparo do profissional para atuar nesse campo, devido às lacunas na grade curricular e falta de estrutura das instituições hospitalares. Portanto, diante dos dados coletados, demonstra-se a necessidade de investimento por parte das instituições hospitalares para treinar seus funcionários, adequar as estruturas e recursos para comportar o tratamento nos cuidados paliativos oncológico. Também é necessário a inclusão de cuidados paliativos como parte do currículo letivo nas instituições educacionais. Além disso é preciso instituir programas de educação permanente aos funcionários, e estruturar a assistência de enfermagem com técnicas de teor humanizado, integrativo e individual aos pacientes e familiares.

Cancer is a pathology characterized by a disordered cell growth that affects organs and tissues of the body. The worsening of the oncological clinical case is called metastasis, where cancer cells spread to other regions of the body. As cancer is a disease with a high fatality rate, there is a great possibility that palliative treatment will become a better option for the full benefit of the cancer client and their families, as soon as the impossibility of cure is presented. This study seeks to identify the best care techniques that can be adopted by the nursing team to provide a humanized and holistic treatment in adult palliative oncological care, highlighting the promotion of well being and quality of life of patients and families. From the collection and review of bibliographic articles collected in the LILACS (Latin American and Caribbean Literature in Health Sciences), VHL (Virtual Health Library) and SciELO (Scientific Electronic Library Online) platforms, which address nursing in palliative care in adult oncology published from 2003 to 2022 in Portuguese. We identified 13 studies that align themselves in the construction of care focused on the comfort and dignity of the patient, as well as the importance of including family members in this treatment, addressing some palliative care techniques that the nursing team can exercise in the hospital environment. In contrast, it was also identified the lack of preparation of the professional to work in this field, due to the gaps in the curriculum and lack of structure of hospital institutions. Therefore, in view of the data collected, it is demonstrated the need for investment on the part of hospital institutions to train their employees, adapt the structures and resources to accommodate the treatment in palliative oncological care. It is also necessary to include palliative care as part of the school curriculum in educational institutions. In addition, it is necessary to institute permanent education programs for employees, and to structure nursing care with humanized, integrative and individual techniques for patients and families.
